Iterated medical assessment form

Although, maybe not appreciated by the busy practitioner and his secretary, when I received my ADHD assessment forms, I couldn't not do something about it.

I found it confusing to figure out what I needed to complete versus what a family member or partner should fill. Let alone actually filling in the form itself.

To iterate, I streamlined the form into sections that are labelled to indicate who should fill them. Additionally, I made the form more accessible by removing inaccessible fonts, colours and amending copy to combat the emphasis the owner was clearly trying to communicate.

Iterated medication request form

Similarly to the previous, I also took the initiative to see what I could do with the medication request form.

I have streamlined the questions, ensuring each query has space for a response - no more questions sharing boxes. Information that is required, is directly requested with clear and plainly written reasoning for these requests.

I would love to have asked the user more questions, specifically with this form, as the content heirarchy is odd. At this point I can only assume the ordering is to enable the receptionist to input provided details efficiently, so I have left this be as much as possible.

Royal Exchange Theatre membership letters

Here are some letters I created as - I believe - an interview task for The Royal Exchange Theatre (RET).

Here I did my best to be brief and informative with my response, taking into account styling, branding and tone of voice used by the RET.

I also submitted a duplicate of the letter that took reasonable adjustments for visual impairments into account.
